To get access to Tatsumaro the Dung Lover in Nioh 3, first you need to finish the side quest Putrid Paradise at the Heian region’s Hachijo area. After this fast travel to the castle and you can see the location of the NPC from the map. But the map clearly doesn’t show where he is above or below the ground in cave.
From that fast travel point head right into a small building and look for a hidden ladder in the room. Climb down the ladder and you will see a blocked path interact with it to reveal the hidden path.
